Accident at work results in death

An accident at work that resulted in a man being crushed to death has led to an engineering firm being penalised to the tune of £100,000.
Paul Thorngate was working at a factory owned by his employer Melloy Limited near Pontypridd when a falling crate of aluminium car parts fell on top of him due to the inadequacy of safety constraints there.
At the time of the incident, the 44-year-old was operating alone in the firm's heat treatment area, meaning there were no colleagues available to help him.
The Health and Safety Executive established in a subsequent investigation that the wire rope hoist supposed to be keeping the equipment suspended failed, while it was also found that Melloy - a firm set up in 2004 - had not conducted a proper risk assessment of this piece of machinery.
Jayne Beer, Mr Thorngate's partner, commented: "Words cannot describe how much of a gap Paul's death has left in all our lives."
